EasableTM Privacy Notice

TABLE OF CONTENTS

Thank you for using easableTM, the remote supplemental disease management and telehealth mobile application (the “App”). This Privacy Notice (“Notice”) describes how data from or about you (“Personal Data”) is collected, used, and shared in connection with the App, which is owned and operated by Pfizer Inc. and its affiliates (“Pfizer”) and administered by Firstview, LLC, and its affiliates (“FirstView”) (collectively, Pfizer and FirstView are “we”, “our”, or “us”). The terms “you” and “user” refer to individuals who use or interact with the App.

We work with your group health plan to provide the App to you, and we have agreed to process your Personal Data in accordance with your health plan’s instructions. You should review your health plan’s HIPAA Notice of Privacy Practices for more information on how your Personal Data, including Personal Data processed in association with using Easable, is used by your health plan.

To the extent you use the telehealth services provided by Truepill, Inc. (“Truepill”) and made available within the App, you may review Truepill’s Notice of Privacy Practices here. If you are prescribed medication by a healthcare provider through the App, and elect to fill that prescription through the App, you may review the applicable dispensing pharmacy’s Notice of Privacy Practices. This Notice is unique to Easable and differs from other Pfizer or FirstView privacy policies and notices. By using the App, you acknowledge and consent to the practices outlined in this Notice.

Personal Data Collected, Purposes, and Recipients

We collect, use, and disclose Personal Data, including your contact information, payment information, health information, and information about your device, browser, and network connection (like IP addresses, device type, device identifier, usage logs, and interactions with the easable App), for the purposes of providing you and your health plan with services, subject to your health plan’s Notice of Privacy Practices. We do not sell or license your Personal Data.

If you use the App to access the telehealth services provided by Truepill, you will be prompted to provide permission for the App to use your device’s camera and microphone. On Android phones, we are required to ask your permission to access your media files; however, we do not process any media or files on your phone outside of information you enter into the App.

Retention

We will retain your Personal Data as instructed by your health plan. If you wish to request deletion of your information, contact your health plan.

Children

The App is not available for download or use, and should not be made available for download or use, to children under the age of 13 (“children”). We do not through this App knowingly collect, maintain, disclose, or otherwise process Personal Data from children.

The App may contain links to other websites or applications that are not owned or operated by us. You should carefully review the privacy policies and practices of these websites or applications before visiting them, as we cannot control and are not responsible for their privacy policies or practices.

Safeguarding Information

We use technical, administrative and procedural measures designed to safeguard your Personal Data from unauthorized access or use. No such measure is ever 100% effective though, so we do not guarantee that your Personal Data will be secure from theft, loss, or unauthorized access or use, and we make no representation as to the reasonableness, efficacy, or appropriateness of the measures we use to safeguard such data. In addition, it is your responsibility to safeguard any passwords, identification or ID numbers, or similar individual information associated with your use of the App.
